Medical Records Reviewed
We specialize in reviewing, condensing and summarizing medical records for your Independent Medical Examination (IME), medical opinion or second opinion examination, disability examination as well as preparation for depsoition and/or trial testimony. In other words, just about any time you receive that huge stack of medical records for review.

All cases are reviewed, condensed and summarized as per your specific requests. The summary categories include Diagnoses/Procedures/Impairmants; Psychosocial History; Current Medical Status; Potential Medical Treatment and Care' (listing) Treating Physicians, "Reasonable and Prudent" care provided; Vocational Issues and Referrals to appropriate expert witnesses. The report content is produced in a straightforward, unbiased format with references listed for any key issues.

A diskette copy is included with your report. Reports can be emailed in several formats including MS Word/Windows Vista, etc. HIPPA confidentiality regulations will be observed at all times.

Records are reviewed by Linda M. Davis, CNLCP, CM (Certified Nurse Life Care Planner and Case Manager). She has 30+ years nursing and healthcare experience, 15 years medical legal consulting. How Do We Start?
Medical records can be picked up and/or delivered to you office or home by bonded and insured courier (FEDEX). This service is billed at the current rate for weight and requested pick-up and/or delivery time.

Office hours for Linda Davis & Associates, LLC are from 9 am to 5 pm, Monday through Friday, Eastern Standard Time - other hours, by prior appointment.

 
Costs
All work requires a retainer. Medical record review, condensing and written reports are billed at our basic rate. Review, condensing and referencing of medical records, depositions and other documents and/or trial testimony are charged separately. Rush work requiring extended hours and/or weekends are billed at a pre-established rate. All billing, invoices and direct expenses (FedEx shipping, etc.) must be paid in full prior to delivery of reports. Flat fees, as well as discounts for longstanding or returning clients are available only by prior arrangement.
